PrePaid Emergency Travel

TERMS & CONDITIONS:   COVERAGE

1. You can select up to 10 friends or family to be covered on this contract that you, the subscriber, can travel to.
2. There is a 90 day waiting period before coverage will begin on covered members.
3. The Company will make all travel confirmations for you.

Conditions Covered: Emergency round-trip transportation by members for the following situations:

1. Death of any contract listed member. Death certificate or physician's statement must be verified
2. Accident of any kind that causes or creates a life-threatening condition to any contract listed member. Confirmation by hospital is required.
3. Massive heart attack suffered by any contract member listed on the contract. Confirmation by hospital is required.
4. Massive stroke suffered by any contract listed member. Confirmation by hospital is required.
5. Any contract member listed that is reported missing over 48 hours. Police report must be verified.
6. Any contract listed member that is a passenger traveling on an airplane that has crashed or a boat that has sunk. Boat must be missing for over 24 hours.
7. Natural disaster that causes life-threatening injury to a contract listed member. Confirmation by hospital is required.
8. Imminent life-threatening health condition occuring to contract listed member. Must not have been discovered by a physician during the 12 months prior to enrollment in the plan. Confirmation by physician is required.
9. Illness suffered by a contract listed member that results in the hospitalization for sudden life-threatening conditions. Confirmation by hospital is required.

TERMS & CONDITIONS:   BILLING

1. A one time signup fee of $74.00, for all plans, when signed up as an individual or family flex.
2. Individual Bereavement & Medical Emergency (covers travel for one registered member for life-threatening accidents or illness and death - travel to 6 people) - a monthly fee of $19.95.

3. Family Flex Bereavement & Medical Emergency (covers travel for one of up to 4 registered members for life-threatening accidents or illness and death - travel to 8 people) - a monthly fee of $19.95.
   • Save 10% and pay only $282 annual.
4. Individual Bereavement Only (covers travel for one person, for death only - travel to 10 people) - a monthly fee of $14.95
   • Save 10% and pay only $228 annual.
5. Family Flex Bereavement Only (covers travel for one of up to 4 registered members for death only - travel to 8 people) - a monthly fee of $14.95.
   • Save 10% and pay only $228 annual.
6. Couple, joint registration and same family member list only (covers travel for two registered family members for life-threatening accident or illness and death - travel to 8 people) - a $128 registration fee and $35.95 monthly fee (discounts applied).
   • Save 10% and pay only $503 annual.
7. Family of three, joint registration and same family member list only (covers travel for three registered family members for life-threatening accident or illness and death - travel to 8 people) - a $192 registration fee and $54.95 monthly fee (discounts applied).

8. Family of four, joint registration and same family member list only (covers travel for four registered family members for life-threatening accident or illness and death - travel to 10 people) - a $256 registration fee and $73.95 monthly fee (discounts applied).

9. Family of five/Group, joint registration only but different family member list ok (covers travel for five registered family members for life-threatening accident or illness and death - travel to 10 people) - a $245 ($49 per person) registration fee and $99.95 monthly fee.
